Accident Notification Box

This PDF template will open as a fillable PDF in Adobe Acrobat. After typing your emergency information in the box, you must SAVE AS a (picture file .JPG) which will allow you to INSERT the filled out graphic into your brochure, map, or document. If you are not able to SAVE AS a .JPG file, try this method - Go to the EDIT menu then select TAKE A SNAPSHOT, click and drag your mouse over the entire graphic to COPY. You will then be able to PASTE the image into your document.

INSERT this JPEG file as a PICTURE directly into your brochure, flyer, or other document. Then INSERT a Text Box over the white space to add in the emergency contact information.
